Santa Rosa Care Center Sells for $30 Million; Ensign Group Acquires Operations

TUCSON, AZ (December 11, 2025) — The 144-bed Santa Rosa Care Center, located at 1650 N. Santa Rosa Avenue in Tucson’s Northeast submarket, sold on December 1, 2025, for $30,000,000, according to Pima County Recorder documents.

The long-established behavioral and medical care facility, built in 1966, totals 39,685 square feet on 2.93 acres, with a price of $755.95 per square foot and $208,333 per bed.

The seller, SR Building LLC, represented by Harvey and Deborah Nevins, transferred the property to Santa Rosa Propco LLC, an entity affiliated with The Ensign Group, Inc. (NasdaqGS: ENSG). Ensign simultaneously acquired the facility’s operations as part of a multi-state portfolio expansion that also included Willow Point Rehabilitation and Nursing Center, The Rehabilitation Center at Sandalwood, and Edgewater Health and Rehabilitation.

Santa Rosa Care Center provides licensed behavioral and rehabilitative services, with off-street parking, an activity center, and a long history of serving Tucson’s medical care community.

“We are excited to add another operation to one of our more mature and thriving markets,” said Barry Port, Ensign’s Chief Executive Officer. “This facility clusters well with other operations in Tucson and we are looking forward to seeing what the future holds,” he continued.

In a separate transaction on the same day, Ensign announced that it acquired the real estate and operations of “Willow Point Rehabilitation and Nursing Center”, a 45-bed skilled nursing facility located in Kansas City, Kansas. The real estate was purchased by a subsidiary of Standard Bearer Healthcare REIT, Inc., Ensign’s captive real estate company, and the property was leased to an Ensign-affiliated operator under a long-term lease effective as of December 1, 2025.

In another transaction on the same day, Ensign announced that it acquired the operations of two facilities in Colorado (i) “The Rehabilitation Center at Sandalwood”, a 103-bed skilled nursing facility located in Wheat Ridge, Colorado, and (ii) “Edgewater Health and Rehabilitation”, a 69-bed skilled nursing facility located in Lakewood, Colorado. An Ensign-affiliated operator operates both facilities and is subject to a long-term triple-net lease. These acquisitions were effective as of December 1, 2025.

These acquisitions bring Ensign’s growing portfolio to 373 healthcare operations, which include 47 senior living operations across 17 states. Ensign subsidiaries, including Standard Bearer, own 156 real estate assets. Mr. Port reaffirmed that Ensign is actively seeking opportunities to acquire real estate and to lease both well-performing and struggling skilled nursing, senior living, and other healthcare-related businesses throughout the United States.

This transaction continues the trend of major healthcare operators expanding their presence in Southern Arizona through the acquisition of both real estate and operating licenses.
